All you need to do is enter your last name and date of birth to begin. The user-friendly interface guides you through each form, including capturing any necessary pictures step by step.

Once you have completed all the required forms, the system will automatically generate an alert in our Practice-Web database. This alert notifies our front desk staff that you are ready to be escorted to your designated treatment area swiftly and efficiently.
